Participant Information and Research Interests Form

Please fill out this Participant Information and Research Interests Form by October 16. Your answers on this form will assist our genealogists in preparing for your consultations! Please note- there is no way to save this form and come back to it later- you may wish to type your answers in a word document and copy them into the form later.

You will receive three consultations as part of the tour- the first of which will be given VIRTUALLY prior to our arrival in Salt Lake City. The other two will be conducted in person, on-site at the FamilySearch Library during the tour.

While using this form is preferable, if you run into any technical issues filling it out you may also submit your answers via email to Kathleen.

Just mention you are staying with the group "New England Historic Genealogical Society." Note: We do not provide a rooming list until 30 days prior to the start of the program, so they may not have your name if you call before then, but they will still be able to assist you.
